Things I've learned from the few monologues I've seen: I'll never get women. I'll only moderately get transwomen when it comes to transition related things. Other'n that, nothing.

Actually someone's gender is predetermined by the genetic material transferred during conception, unless an abnormality forms which prevents the child from developing normally and becomes transgendered...in which case a gender would be assigned "the day they were born."
Just wanted to point that out.

It means assigned socially/legally/etc, and even intersexed have to deal with that. Some poor folks actually get surgery as babies to make them "normal" only to grow up and have it turn out that they'd have been happier with how they were born. At least with people born biologically "normal" you don't find out that anyone cut on you to make you like that, it was just an unfortunate accident.
And back to the social/legal/whatever assigned: While gender likely does have genetic causes, we aren't aware enough of how to tell these genetic causes to say at birth "yep, this one is definitely a neutrois/androgyne/boy/girl/etc". Even if we could, it would be wrong to do anything to make them like that until the child can choose what they want to do.

Yeah, the plight of transgendered people is a sad one, and it is very difficult to properly "assign" them genders when they're born. We should never go meddling in something we don't fully understand.

Sex assignment is not random. Unless you're a herm, it's based on your physical makeup.
